Club leader (or advisor for independent member) must sign on the line after last entry and send form(s) to County Extension Office once tagging is complete. Extension Office will confirm member’s status by signing original form, then will forward original to the State 4-H Office. County office will give an official copy to exhibitor. 4-H member must have the animal approval form available at all times during an event. Market animals will have an electronic chip as well as an ear tag placed by a designated team. The chip & tag will be placed during January for market steers. A DNA sample will also be taken at the time of tagging. This form is to certify that the above exhibitor is a bonafide 4-H member and the animal (s) listed are eligible to be shown by this member in current year 4-H animal shows.

Animals to be sold at Windsor Fair and Fryeburg Fair must be listed on this form. Once chip and ear tag are placed, this form must be submitted to your 4-H Extension Staff by deadlines listed on the front of this form. An embossed (official) copy of this form with a 4-H Extension Staff’s signature must be presented to the Superintendent of the 4-H show in which you participate, and must agree with information on the State 4-H Database.
