2009 PINE TREE STATE 4-H FOUNDATION
POST-SECONDARY EDUCATION SCHOLARSHIPS
APPLICATION FORM
Application Deadlines: Applications must be received in your county 4-H office by
March 6, 2009. County 4-H Staff must send approved applications to the Foundation Office to be received by March 13, 2009.
Eligibility: The qualified candidate must be: 1) a current member of 4-H (enrolled by December 31, 2008) who is currently active in 4-H activities and 2) a graduating senior or an individual who has graduated from high school, but has delayed going to college for no more than one year. Candidates are only eligible to apply in the spring prior to entering college.
Scholarship Awards: The Pine Tree State 4-H Foundation will award nine post-secondary education scholarships in May 2009. Scholarships are awarded based on a combination of demonstrated academic and 4-H excellence. Five of the scholarships will be awarded to the top applicants from Maine and one will be awarded to a fifth top candidate who may be a Maine or New Hampshire 4-Her. The Foundation's 2009 post-secondary education scholarships are:
· Claude C. Clement Scholarship (2 - $1,000) - Maine 4-Her
· Verna Church Witter Scholarship ($1,000) - Maine 4-Her
· Azure Dillon 4-H Memorial Scholarship ($1,000) - Maine 4-Her
· Evelyn L. Trotzky Scholarship ($1,000) - Maine 4-Her
· Farm Credit of Maine Scholarship ($1,000) – Students currently enrolled at University of Maine in College of Agriculture. Maine 4-Her planning to continue their post-secondary education at a Maine institution whose objective is to major in a field related to commercial farming, fishing, or forest products or in another field with the intent to work in businesses related to these industries.
· Doris Roberts, H. Lawrence, and George Lawrence Humphrey 4-H Scholarship ($500 – Maine 4-Her
Receiving the Scholarship: Recipients will be requested to attend the Foundation’s Annual Meeting on May 14, 2009, to receive their scholarship in person.
Payment Requirements: Scholarships are payable upon successful completion of the first semester of school. To receive the scholarship funds, recipients must mail a completed first semester transcript to the Foundation Office. A check, made payable to the recipient, will be sent by the Foundation within two weeks of receiving the transcript.

When applying, please do not send ribbons, trophies, certificates, etc. as part
of your application. If you are applying for an award, please send black and
white photocopied versions of your 4-H story and resume to us and keep the
original either at your county office or for yourself.

Mail your complete application to the 4-H STAFF AT YOUR COUNTY COOPERATIVE EXTENSION OFFICE. In order to ensure equity among applicants, incomplete applications will not be considered in the review process.
